Insurance Coverage

Many lactation visits are covered at 100% by the following plans:

• United Healthcare/UMR

• Aetna

• Cigna

• Colorado Medicaid

• First Health

Who Can Book Immediately

If you have United, or Aetna your visit is typically covered at 100%.
You can book your appointment without verification.
Your only out-of-pocket cost is the travel fee for in-home visits.

Who Needs to Verify Coverage

If you have Cigna or First Health, please complete the appropriate coverage verification first by clicking the verification button.
Once verified, you may book your appointment.

Travel Fee (In-Home Visits)
In-home visits are available within 30 minutes of Lafayette. A flat $50 travel fee applies to all in-home visits.

Medicaid clients: All Colorado Medicaid clients must be seen in-office.

Colorado Medicaid

Colorado Medicaid visits are covered at 100% and must be scheduled in-office.
A referral is required from the provider linked to the active Medicaid coverage (either the lactating parent’s provider or the baby’s provider). The referral must include the provider’s NPI number and signature, and can be faxed to 720-677-0263.
